Bortenat-3.5mg Description


Bortenat 3.5mg injection is indicated for the treatment of blood plasma cell cancer (multiple myeloma) in patients with or without a before history of treatment and mantle lymphoma.

Bortenat 3.5mg inhibits the growth of cancer cells, and then eventually destroyed by the body. It will effect the growth of normal body cells, other effects also occur.


Uses of Bortenat-3.5mg


 Bortenat 3.5mg injection is used to treat the multiple myeloma in patients

Bortenat 3.5mg injection is also indicated for the patients suffering from mantle cell lymphoma, who has been used at least 1 before treatment.

Side effects of Bortenat-3.5mg


 Bortenat 3.5mg injection causes some effects in patient, In such situation, you should seek to the doctor immediately.

How to use Bortenat-3.5mg


Adult dose for lymphoma

Previously without treating mantle cell lymphoma dose:

1.3 mg/m2 ( bolus IV injection) twice weekly

Bortenat  Co administration with, cyclophosphamide, doxorubicin, and oral prednisone, IV rituximab for two weeks  i.e days 1, 4, 8, and 11 followed by a ten day rest period (days 12 through 21).

Treatment for relapsed mantle cell lymphoma

Regular dose: 1.3 mg/m2 which is administrated bolus IV injection or subcutaneously (SC) two times in  weekly for two weeks i.e  days 1, 4, 8, and 11 followed by a ten day rest period (days 12 through 21).

Treatment continuing beyond 8 cycles  may be given one day in weekly for 4 weeks  i.e days 1, 8, 15, and 22, followed by a 13-day rest (days 23 through 35).

Adult dose for multiple myeloma:

Previously without treating multiple myeloma dose:

1.3 mg/m2 which is administrated as 3 to 5 bolus IV injection or SC combine with oral melphalan and oral prednisone for 9 to 6 weeks therapy cycles.

Bortenat  in 1 cycles through 4 cycles  administrated  two times in  weekly i.e days 1, 4, 8,11,22,25,29 and 32  and in  5 cycles through 9 cycles is administrated once weekly i.e days 1,8,22 and 29)

Treatment for relapsed multiple myeloma:

Regular dose: 1.3 mg/m2 which is administrated bolus IV injection or subcutaneously (SC) two times in  weekly for two weeks i.e  days 1, 4, 8, and 11 followed by a ten day rest period (days 12 through 21).

OVERDOSE: Bortenat has no specific overdosage, In human when administrated more than two times then the prescribed dose the it will causes symptomatic hypotension and thrombocytopenia. If occurs patients should monitored and give appropriate supportive care.

How to Bortenat-3.5mg works


Bortenat 3.5mg injection used in mammalian cells is a reversible inhibitor of the chymotrypsin-like activity of the 26-S proteasome. It inhibits targeted proteolysis which  affects multiple signaling cascades responsible for normal homeostatic functions within the cell, leading to cell death.

Absorption: The peak plasma concentration after 1st dose is 57 and 112ng/mL, If administrated  two times a week, Cmax observed for 1mg/m2dose is 67 to 106ng/mL, for 1.3mg/m2 is 89 to 720ng/mL.

Distribution: human serum plasma concentration is >80%

Metabolism: Bortenat 3.5mg injection metabolized mainly through cytochrome P450 isoenzymes CYP3A4, CYP2C19 & CYP1A2.

Elimination: The excretion pathways of Bortenat have not been characterized.

The terminal elimination of half life after the 1mg/m2 dose is 40hrs - 193hrs, after dose 1.3mg/m2 is 76hrs - 108hrs.

  1. Inform the doctor if you are allergic to bortezomib, any other regimens, or any of the ingredients in bortezomib capsules.
  2. Take into the knowledge that bortezomib may reduce the  fertility in men and women. It not means that you or your partner cannot become pregnant. Inform doctor before if you are pregnant or plan to become pregnant. After become pregnant, you should use birth control while  treatment with bortezomib and for at least 2 weeks after your treatment, become pregnant during bortezomib therapy, call your doctor immediately. bortezomib may harm the fetus.
  3. Inform the doctor if you are breastfeeding or plan to breastfeed. Avoid breastfeed during bortezomib treatment.
  4.  If you had any surgery, like dental surgery, tell to your doctor before starting therapy.
  5.  Bortezomib may cause dizziness, lightheadedness, and fainting on getting up in a lying position.  Commonly, People who have fainted in the past, dehydrated, and taking medications which lower blood pressure. To avoid this problem, get up from bed slowly,  keep your feet  resting on the floor for a few minutes piror to  standing up.

Bortenat 3.5mg injection interaction with alcohol is unknown. Please consult with the doctor.

Pregnancy

Category D: while administrating Bortenat 3.5mg injection in pregnant women, positive evidence on fetal risk depends on human studies but potential benefits by using the drug may have common risk against fetus.

Lactation

Bortenat 3.5mg injection is excreted into breast milk taking account on this, discussion should done on whether to discontinue the breast feeding or the drug. While using the drug is unsafe during breast feeding to their infants

Driving

Bortenat 3.5mg injection will affect the ability of driving due to its side effects like drowsy, dizzy, or lightheadedness or fainting, blurred vision. Avoid driving unless you feeling well.


Kidney

Bortenat 3.5mg injection used with caution in patients with severe renal disease, dose adjustment is required and limited data is available on the tablets in mild, moderate kidney problem, no dose adjustment is necessary .Please consult doctor.

Liver

The patient with severe liver disease should Bortenat 3.5mg injection with caution. Limited information of the drug in mild to moderate liver disease. dose adjustment is needed. Please consult with doctor,



Missed Dosage

If any doses missed by you, administer the dose as soon as possible you remember, If it is near to next dose then skip the missed dose and follow the regular schedule. Avoid taking 2 dose or extra dose at a time. If you not able to follow regular dose schedule, keep an alarm to remind your dose schedule. Please consult the doctor for any dose schedule changes or new schedule for equally missed dose
