Uses of Hepcvir L Tablet

Hepcvir L (Ledipasvir) is a type of antiviral medication is called HCV NS5A inhibitors, which blocks the spread of hepatitis virus in the body and Sofosbuvir  means nucleotide polymerase which will reduce the amount of hepatitis C virus in the body. Both drugs combination is used alone or with ribavirin for the treatment of certain types of chronic Hepatitis C in adults and childerns of 12years or weighing of 35kg.

How to use Hepcvir L Tablet

  • Hepcvir L  tablet is indicated for chronic hepatitis C viral infection.

  • In adult

         Dose recommended is ledipasvir  90mg and Sofosbuvir 400mg of one tablet, once in a day. Hepcvir L can be given with or without food for 12weeks duration.

For genotype1,4,5 and 6 : Treatment of new or experienced patients without cirrhosis or with compensated cirrhosis used with coexistent ribavirin in eligible patients for 12 weeks.

For genotype 1 and 4 : Liver transplant recipients for new or experienced patients without cirrhosis or with compensated cirrhosis used with coexistent ribavirin in eligible patients for 12 weeks.


  • Pediatric dose

           1 tablet of  Hepcvir L 90m/400mg once daily for children and adolescents ≥ 12 years or weight ≥35kg.

Overdose: Hepcvir L overdose has no specific antidote available.If occurs should monitored for evidence of toxicity or seek to emergency medical attention or call the poison help line Contact 1-800-222-1222.

How to Hepcvir L Tablet works:

Hepcvir L (Sofosbuvir 400mg) is inhibitor of nucleotide polymerase, which especially prevent  HCV NS5B (non-structural protein 5B) RNA-dependent RNA polymerase. It form the pharmacologically active uridine analog triphosphate (GS-461203) in intracellular metabolism, sofosbuvir integrate  into HCV RNA by the NS5B polymerase and acts as a chain terminator . Hence mainly action of  Sofosbuvir inhibits the HCV viral proliferation by linking  to the two Mg2+ ions present in HCV NS5B polymerase's .

Hepcvir L( Ledipasvir 90mg) is an Hepatitis C Virus (HCV) NS5A protein inhibitors which  needed for the replication of viral RNA and  HCV virions assembly.It is predicate to interfere with hyperphosphorylation of NS5A protein inhibitors which is needed for viral production.


  • Absorption: Well absorbed and peak plasma concentration of ledipasvir is 4 – 4.5 hr post dose and Sofosbuvir is 0.8 -1 he post dose.

  • Distribution: Plasma protein binding of ledipasvir >99.8% and Sofosbuvir is 61% to 65%

  • Metabolism: Ledipasvir metabolism was observed by human is CYP1A2,CYP2C8,CYP2C9,CYP2C19,CYP2D6,CYP3A4

Primarily Sofosbuvir mobilized in the liver in the forms of pharmacologically active nucleoside analog triphosphate GS-461203.

  • Excretion: Ledipasvir elimination via feces (-86%) and urine (1%) and for Sofosbuvir via urine (80%) and feces (14%).

  • Half life elimination of Ledipasvir is 47hrs and Sofosbuvir is -0.5hrs.

Expert Advice for Hepcvir L Tablets

  • Before taking tablet you should inform doctor or pharmacist, If you are allergic by ledipasvir or Sofosbuvir or other medication  .
  • When amiodarone taken along with Sofosbuvir which is  used for serious irregular heartbeats. Consult your doctor because the risk of slow heartbeat may be increased.
  •  Avoid pregnancy in pregnant women or  male partner  and take extreme care when on treatment with sofosbuvir.
  • This tablet should be used only in co administration  with other antiviral drug ( Ribavirin) or with Pegylated interferon and Ribavirin.
  • Sofobuvir treatment is not prescribed in patients with severe Renal or hepatic dysfunctions or depression.

Pregnancy category B: In studies of  Animal reproduction that  have been failed to described a risk to the fetus and have been shown Interferon’s have Abortifacient effects, there are no adequate and well-controlled studies in pregnant women. Hepcvir L 90/400mg with Ribavirin will affect the Birth Defects and Fetal Death. Avoid in pregnant women and male partner. Patients should check Negative Pregnancy Test before starting the treatment. Use 2 Effective Methods Of Contraception and check monthly Pregnancy Tests.


Hepcvir L 90mg/400mg amount in human milk is very low and is highly bound to maternal plasma proteins. If mother require this drug then its not necessary to discontinue the drug. May be some doctor prescribe against breast feeding when its combine with ribavirin.


While using of Hepcvir L 90mg/400mg with combination of other antiviral drug will cause dizziness, blurred vision and may affect the ability to drive. Avoid using Hepcvir L during driving until you are feeling well.


Patient with severe kidney disease, Hepcvir L 90mg/400mg should be used with caution and dose adjustment is required. Check up with doctor. When mild to moderate kidney disease, Hepcvir L 90mg/400mg do not require dose adjustment in those patients.


Mild, moderate or severe liver problem in patients have no dose adjustment, safe to use Hepcvir L 90mg/400mg with liver disease. But patient with decompensated cirrhosis using this drug plus ribavirin then clinical and hepatic monitoring is recommended.

Missed Dosage

As soon as you remember take the missed dose. If time for next dose then skips the missed dose and follow the routine scheduled. Do not take more than one tablet per day. Please consult your doctor for further details.
