Side effects of Xeloda
Common side effects:

How to use Xeloda
Adult dose for colorectal cancer
First line treatment for the patients with metastatic colorectal cancer when treated with fluoropyrimidine alone is preferred dose is 1250 mg/m2 given orally twice daily (morning or evening) administrated as 3 week cycles ( 2 weeks followed by a l week rest period)
In patients with DUKES C colon cancer by adjuvant treatment preferred dose is 1250 mg/m2 given orally twice daily (morning or evening) administrated as 3 week cycles ( 2 weeks followed by a l week rest period) for a 8 cycles total ( 24 weeks).
Adult dose for breast carcinoma
The prescribed dose is 1250 mg/m2 given orally twice daily (morning or evening) administrated as 3 week cycles ( 2 weeks followed by a l week rest period) as a 1 hour IV infusion with combination of docetaxel 75mg/m2.
In combination with docetaxel:
The prescribed dose is 1250 mg/m2 given orally twice daily (morning or evening) administrated as 3 week cycles ( 2 weeks followed by a l week rest period).
OVERDOSAGE: Acute overdose with xeloda will includes vomiting, diarrhea, gastrointestinal irritation and bleeding. If an overdose suspected, seek emergency medical attention.
How to Xeloda works
Converstion of capecitabine to 5- fluorouracil (5- FU) is carried out by an enzymes. Normal cells and cancer cells are metabolized in 5- FU to FdUMP ( 5- fluoro- 2- deoxyuridine monophosphate and FUTP ( 5- fluorouridine triphosphate). By two different mechanism the metabolites cause cell injury. Firstly to form a covantly bound ternary complex the FdUMP and the folate cofactor, N5- 10-methylenetetrahydrofolate will bind to thymidylate synthase. By this binding, It may inhibits the formation of thymidylate from 2- deoxyuridylate. Thymidylate is an important messenger of thymidine triphosphate that is essential for the DNA synthesis. Hence the compound deficiency will blocks the cell division.
Secondly, mistakenly FUTP is incorporate in place of uridine triphosphate (UTP)during RNA synthesis. Additionally, FUTP interferes w/ RNA processing and protein synthesis by metabolic error.
Absorption: Readily absorbed. Food decrease rate and extent of absorption. peak plasma concentration is 1.5hr.
Distribution: Plasma protein binding: <60%, mainly to albumin.
Metabolism: capecitabine hydrolysed in the liver into 5'-deoxy-5-fluorocytidine (5'-DFCR) then to 5'-deoxy-5-fluorouridine (5'-DFUR), and subsequently in body tissues into 5-fluorouracil by thymidine phosphorylase enzyme.
Excretion: Via urine (96%, (<3% as unchanged drug) and faeces (<3%). Elimination half-life is 0.75 hr.
